- Drop in to our clinics. You will be evaluated and scheduled for a visit based on the urgency of your symptoms.

- If you have received services at the Tampa or Sarasota-Manatee clinics before, you may send a secure message through patient portal

New patients: please have a scheduled appointment and use the fax number below or bring in your
records at the time of your appointment. Please reference type of appointment (i.e.
general medicine, sexual health, gynecology, dermatology, psychiatry, nutrition, etc.)

Existing patients: may submit records via MyChart secure patient portal or send via Fax.
